14         On page 13, between lines 16 and 17,

17         (14)  This section applies only to a major medical

18  expense health insurance policy as defined in s. 627.643(2)(e)

19  which is offered by a group or an individual health insurer

20  licensed under chapter 624, including a preferred provider

21  policy under s. 627.6471 and an exclusive provider

22  organization under s. 627.6472.
