14         On page 148, between lines 21 & 22,

16  insert:

17         Section 126.  Subsection (8) of section 400.802,

18  Florida Statutes, is created to read:

19         (8)  There is established within the Agency for Health

20  Care Administration a pilot project entitled "Pathways to

21  Care".  The pilot project shall utilize a 100-bed facility,

22  the Lake Mary Senior Resource Center, located in Seminole

23  County, Lake Mary, Florida. The Agency for Health Care

24  Administration shall conduct a study in conjunction with

25  Pathways to Care, Inc. of health care services provided to the

26  uninsured and underinsured of Central Florida, providing short

27  term residential recuperative and supportive services, and

28  primary care clinic services. The pilot project shall provide

29  care for the homeless and indigent who require a hygienic,

30  secure environment to recuperate from illness and injury.

31  Preference in participation shall be given to the homeless

  1  patients being discharged from the two hospitals in the area

  2  that provide the largest percentage of uncompensated care.

  3  The clients of the pilot project will be served by case

  4  management and social service interventions, provide for the

  5  location of permanent housing, job-training and employment.

  6  The establishment of this pilot project shall be exempt from

  7  the provisions of s. 408.036(1), Florida Statutes. The pilot

  8  project shall be governed in accordance with the licensing

  9  provisions of this section. The Agency for Health Care

10  Administration shall monitor the pilot project and report its

11  findings to the Governor, the President of the Senate, and the

12  Speaker of the House two years after the program becomes

13  operational.  The report shall contain findings with respect

14  to the impact that the granting of such exemptions had on

15  competition in the health planning district as defined in s.

16  408.036, Florida Statutes, cost to providers, patients, and

17  payors; and the use and occupancy rates of acute care beds in

18  the health planning district;  the impact of securing

19  employment of clients; the types of services received;

20  itemized cost of the services; the source of funding that pay

21  for services, including the proportional share when more than

22  one funding source pays for service; and the total population

23  served.

24         (a)  Within 60 days after the effective date of this

25  act, the agency must amend the state Medicaid plan and request

26  any federal waivers necessary to implement and fund the pilot

27  program.

28         (b)  The provisions of this pilot program relating to

29  transitional care and social services shall be implemented to

30  the extent available appropriations contained in the annual

31  General Appropriations Act are specifically designated for the

12         requiring a report to the Legislature and

13         Governor with findings including cost to

14         providers, patients and payors, requesting AHCA

15         to file necessary waivers;
