14         On page 6, between lines 18 and 19,

16  insert:

17         (c)  For noninstitutional providers, all claims must be

18  mailed or electronically transferred to an insurer within 90

19  days after completion of the service and after the provider

20  has been furnished with the correct name and address of the

21  patient's insurer. For institutional providers, unless

22  otherwise agreed to through contract, all claims must be

23  mailed or electronically transferred to an insurer within 90

24  days after completion of the service and after the provider

25  has been furnished with the correct name and address of the

26  patient's health insurer.
