13         Amendment (with title amendment) 

14         On page 42, lines 9-14,

15  remove:  all of said lines,

17  and insert:  under chapter 458 or chapter 459, and the

18  organization determine that a subscriber requires examination

19  by a licensed optometrist or a licensed ophthalmologist for

20  medically necessary, contractually covered services, then the

21  organization shall authorize the contracted primary care

22  physician to send the subscriber to a contracted licensed

23  optometrist or a contracted licensed ophthalmologist.
