14         On page 42, lines 11-14,

15  remove:  all of said lines,

17  and insert:  by a licensed optometrist or a licensed

18  ophthalmologist for medically necessary, contractually covered

19  services, then the organization shall authorize the contracted

20  primary care physician to send the subscriber to a contracted

21  licensed optometrist or a contracted licensed ophthalmologist.
