13         Amendment (with title amendment) 

14         On page 42, between lines 14 & 15,

16  insert:  Subsection (8) of section 400.802, Florida Statutes,

17  is created to read:

18         (8)  There is established within the Agency for Health

19  Care Administration a pilot project entitled "Pathways to

20  Care".  The pilot project shall utilize a 100-bed facility,

21  the Lake Mary Senior Resource Center, located in Seminole

22  County, Lake Mary, Florida. The Agency for Health Care

23  Administration shall conduct a study in conjunction with

24  Pathways to Care, Inc. of health care services provided to the

25  uninsured and underinsured of Central Florida, providing short

26  term residential recuperative and supportive services, and

27  primary care clinic services. The pilot project shall provide

28  care for the homeless and indigent who require a hygienic,

29  secure environment to recuperate from illness and injury.

30  Preference in participation shall be given to the homeless

31  patients being discharged from the two hospitals in the area

  1  that provide the largest percentage of uncompensated care.

  2  The clients of the pilot project will be served by case

  3  management and social service interventions, provide for the

  4  location of permanent housing, job-training and employment.

  5  The establishment of this pilot project shall be exempt from

  6  the provisions of s. 408.036(1), Florida Statutes. The pilot

  7  project shall be governed in accordance with the licensing

  8  provisions of this section. The Agency for Health Care

  9  Administration shall monitor the pilot project and report its

10  findings to the Governor, the President of the Senate, and the

11  Speaker of the House two years after the program becomes

12  operational.  The report shall contain findings with respect

13  to the impact that the granting of such exemptions had on

14  competition in the health planning district as defined in s.

15  408.036, Florida Statutes, cost to providers, patients, and

16  payors; and the use and occupancy rates of acute care beds in

17  the health planning district;  the impact of securing

18  employment of clients; the types of services received;

19  itemized cost of the services; the source of funding that pay

20  for services, including the proportional share when more than

21  one funding source pays for service; and the total population

22  served.

23         (a)  Within 60 days after the effective date of this

24  act, the agency must amend the state Medicaid plan and request

25  any federal waivers necessary to implement and fund the pilot

26  program.

27         (b)  The provisions of this pilot program relating to

28  transitional care and social services shall be implemented to

29  the extent available appropriations contained in the annual

30  General Appropriations Act are specifically designated for the

31  purposes contained within the pilot program.

  9         requiring a report to the Legislature and

10         Governor with findings including cost to

11         providers, patients and payors, requesting AHCA

12         to file necessary waivers;
