18 (2) Each community college, college, and university in
19 this state must require all students participating in
20 intercollegiate athletic competition or who are candidates for
21 an intercollegiate athletic team to satisfactorily pass a
22 medical evaluation each year prior to participating in
23 intercollegiate athletic competition or engaging in any
24 practice, tryout, workout, or other physical activity
25 associated with the student's candidacy for an intercollegiate
26 athletic team. Such medical evaluation can only be
27 administered by a practitioner licensed under the provisions
28 of chapter 458, chapter 459, chapter 460, or s. 464.012, and
29 in good standing with the practitioner's regulatory board. The
30 Florida Board of Education shall establish requirements for
31 eliciting a student's medical history and performing the

1 medical evaluation required under this subsection, which shall
2 include minimum standards for the physical capabilities
3 necessary for participation in intercollegiate athletic
4 competition as contained in a uniform preparticipation
5 physical evaluation form. The evaluation form shall provide a
6 place for the signature of the practitioner performing the
7 evaluation with an attestation that each examination procedure
8 listed on the form was performed by the practitioner or by
9 someone under the direct supervision of the practitioner. The
10 form shall also contain a place for the practitioner to
11 indicate if a referral to another practitioner was made in
12 lieu of completion of a certain examination procedure. The
13 form shall provide a place for the practitioner to whom the
14 student was referred to complete the remaining sections and
15 attest to that portion of the examination. Practitioners
16 administering medical evaluations pursuant to this section
17 must know the minimum standards established by the Florida
18 Board of Education and certify that the student meets the
19 standards. If the practitioner determines that there are any
20 abnormal findings in the cardiovascular system, the student
21 may not participate unless a subsequent EKG or other
22 cardiovascular assessment indicates that the abnormality will
23 not place the student at risk during such participation.
24 Results of such medical evaluation must be provided to the
25 school. No student shall be eligible to participate in any
26 intercollegiate athletic competition or engage in any
27 practice, tryout, workout, or other physical activity
28 associated with the student's candidacy for an intercollegiate
29 athletic team until the results of the medical evaluation
30 verifying that the student has satisfactorily passed the
31 evaluation have been received and approved by the school. The

1 Florida Board of Education shall promulgate rules necessary to
2 administer this section.
3 (3) Notwithstanding the provisions of subsection (2),
4 a student may participate in intercollegiate athletic
5 competition or be a candidate for an intercollegiate athletic
6 team if the parent or guardian of the student objects in
7 writing to the student undergoing a medical evaluation because
8 such evaluation is contrary to his or her religious tenets or
9 practices. A student who participates without the required
10 medical evaluation pursuant to this
11
