14         On page 7, lines 24-30,

15  remove:  all of said lines,

17  and insert:  licensed physician or licensed dentist who

18  provides the medical service benefits or procedures which are

19  within the scope of a licensed physician's or licensed

20  dentist's license. Any limitation or condition placed upon

21  payment to, or upon services, diagnosis, or treatment by, any

22  licensed physician or licensed dentist shall apply equally to

23  all licensed physicians or licensed dentists, respectively,

24  without unfair discrimination to the usual and customary

25  treatment procedures of any class of physicians or dentists.
