1 A bill to be entitled
2 An act relating to maintenance of official
3 records; providing a process for removal from
4 official records of specified armed forces
5 military-separation forms upon the request of a
6 veteran of the United States Armed Forces or a
7 specified representative thereof; providing
8 requirements with respect thereto; requiring
9 the county recorder to provide specified
10 written notice; providing an effective date.

12 Be It Enacted by the Legislature of the State of Florida:

14 Section 1. Any veteran of the United States Armed
15 Forces or his or her widow or widower, or the attorney,
16 personal representative, executor, or court-appointed guardian
17 of a veteran of the United States Armed Forces at the request
18 of and on behalf of the veteran, has the right to request that
19 a county recorder remove from the official records of the
20 county any of the following forms recorded before, on, or
21 after the effective date of this act: DD-214; DD-215; WD AGO
22 53; WD AGO 55; WD AGO 53-55; NAVMC 78-PD; and NAVPERS 553. The
23 request for removal must specify the identification page
24 number of the form to be removed. The request shall be made in
25 person and with appropriate identification to allow
26 determination of the identity of the veteran making the
27 request or the identity of the person making the request on
28 behalf of a veteran as authorized under this act. The county
29 recorder has no duty to make any inquiries beyond those
30 required to verify the identity of the veteran requesting
31 removal or the person requesting removal on behalf of a

1 veteran. No fee shall be charged for the removal. When the
2 request for removal is made, the county recorder shall provide
3 a written notice to the requesting party that the removal of
4 the document from the official records is permanent and no
5 further record of the document will exist in the official
6 records of the county.
7 Section 2. This act shall take effect upon becoming a
8 law.
