18         (12)  If a contracted primary care physician, licensed
19  under chapter 458 or chapter 459, demonstrates the medical
20  necessity to refer and the organization determine that a
21  subscriber for requires examination by a licensed
22  ophthalmologist or licensed optometrist for medically
23  necessary, contractually covered services, then the
24  organization shall authorize the contracted primary care
25  physician to send the subscriber to a the appropriate
26  contracted licensed provider ophthalmologist.
