15	Remove line(s) 621-636, and insert:
16	Section 14. Subsection (40) is added to section 641.31,
17	Florida Statutes, to read:
18	641.31 Health maintenance contracts.--
19	(40) Every health maintenance organization shall make
20	available to its subscribers the estimated co-pay, co-insurance,
21	or deductible, whichever is applicable, for any covered service,
22	the status of the subscriber's maximum annual out-of-pocket
23	payments for a covered individual or family, and the status of
24	the subscriber's maximum lifetime benefit. Each health
25	maintenance organization shall, upon request of a subscriber,
26	provide an estimate of the amount the health maintenance
27	organization will pay for a particular medical procedure or
28	service. The estimate may be in the form of a range of payments
29	or an average payment. A health maintenance organization that
30	provides a subscriber with a good faith estimate is not bound by
31	the estimate.
