16	(41) Every health maintenance organization shall make
17	available to its subscribers the estimated co-pay, co-insurance,
18	or deductible, whichever is applicable, for any covered service,
19	the status of the subscriber's maximum annual out-of-pocket
20	payments for a covered individual or family, and the status of
21	the subscriber's maximum lifetime benefit. Each health
22	maintenance organization shall, upon request of a subscriber,
23	provide an estimate of the amount the health maintenance
24	organization will pay for a particular medical procedure or
25	service. The estimate may be in the form of a range of payments
26	or an average payment. A health maintenance organization that
27	provides a subscriber with a good faith estimate is not bound by
28	the estimate.
