11  Senator Fasano moved the following amendment:

13         Senate Amendment 
14         On page 3, lines 8-18, delete those lines
15  
16  and insert:  
17         (f)  The patient, for purposes of checking the
18  information for accuracy and reporting any inaccuracies to the
19  department for correction if verified. The Department of
20  Health shall adopt, by rule, the process to be used to verify
21  requested corrections. A patient may request from the
22  Department of Health a copy of any record contained in the
23  electronic prescription monitoring system relating to that
24  patient by providing a written request to the Department of
25  Health and verifying the patient's identity, as required in
26  rule of the Department of Health, and in accordance with any
27  applicable state or federal law. The costs associated with the
28  administration of this paragraph shall be recouped as provided
29  by rule of the Department of Health.
