15	Between lines 719 and 720, insert:
16	Section 10. Paragraph (e) of subsection (2) of section
17	381.7353, Florida Statutes, is amended to read:
18	381.7353 Reducing Racial and Ethnic Health Disparities:
19	Closing the Gap grant program; administration; department
20	duties.--
21	(2) The department shall:
22	(e) Coordinate with existing community-based programs,
23	such as chronic disease community intervention programs, cancer
24	prevention and control programs, diabetes control programs, oral
25	health care programs,the Healthy Start program, the Florida
26	KidCare Program, the HIV/AIDS program, immunization programs,
27	and other related programs at the state and local levels, to
28	avoid duplication of effort and promote consistency.
29	Section 11. Paragraph (a) of subsection (2) of section
30	381.7355, Florida Statutes, is amended to read:
31	381.7355 Project requirements; review criteria.--
32	(2) A proposal must include each of the following
33	elements:
34	(a) The purpose and objectives of the proposal, including
35	identification of the particular racial or ethnic disparity the
36	project will address. The proposal must address one or more of
37	the following priority areas:
38	1. Decreasing racial and ethnic disparities in maternal
39	and infant mortality rates.
40	2. Decreasing racial and ethnic disparities in morbidity
41	and mortality rates relating to cancer.
42	3. Decreasing racial and ethnic disparities in morbidity
43	and mortality rates relating to HIV/AIDS.
44	4. Decreasing racial and ethnic disparities in morbidity
45	and mortality rates relating to cardiovascular disease.
46	5. Decreasing racial and ethnic disparities in morbidity
47	and mortality rates relating to diabetes.
48	6. Increasing adult and child immunization rates in
49	certain racial and ethnic populations.
50	7. Decreasing racial and ethnic disparities in oral health
51	care.
