11  Senators Bennett and Wasserman Schultz moved the following
12  amendment:

14         Senate Amendment (with title amendment) 
15         On page 2, lines 1-19, delete those lines
16  
17  and insert:  spontaneous fetal demise occurring after a
18  gestation period of less than 20 completed weeks must notify
19  the mother of her option to arrange for the burial or
20  cremation of the fetal remains, as well as the procedures
21  provided by general law. Notification may also include other
22  options such as, but not limited to, a ceremony, a
23  certificate, or common burial of the fetal remains.
24         (3)  The Department of Health shall adopt rules to
25  develop forms to be used for notifications and elections by
26  the health care practitioner, and the health care practitioner
27  shall provide the forms to the mother.
28         (4)  A facility licensed pursuant to chapter 383 or
29  chapter 395 having custody of fetal remains following a
30  spontaneous fetal demise occurring after a gestation period of
31  less than 20 completed weeks must notify the mother of her
