13         Senate Amendment 
14         On page 6, line 2, after the period,
15  
16  insert:  A practitioner writing a prescription and a
17  pharmacist dispensing a medication according to the
18  prescription may not be required to disclose the prescribing
19  practitioner's federal Drug Enforcement Administration
20  identification number unless the prescription is for a
21  controlled substance listed on Schedule II, Schedule III,
22  Schedule IV, or Schedule V.
