14          Amendment (with directory and title amendments)
15          Between lines 1214 and 1215, insert:
16          (40) Each health maintenance organization shall make
17    available to a contract holder as part of the application for
18    any such contract, for an appropriate additional premium, the
19    benefits or level of benefits specified in the December 1999
20    Florida Medicaid Therapy Services Handbook for genetic or
21    congenital disorders or conditions involving speech, language,
22    swallowing, and hearing and a hearing aid and earmolds benefit
23    at the level of benefits specified in the January 2001 Florida
24    Medicaid Hearing services Handbook.
25          (a) Such optional coverage is not required to be offered
26    when substantially similar benefits are included in the contract
27    issued to the subscriber.
28          (b) This subsection does not require or prohibit the use
29    of a provider network.
30          (c) This subsection does not prohibit an organization from
31    requiring prior authorization for the benefits under this
32    subsection.
33          (d) This subsection does not apply to health maintenance
34    organizations issuing individual coverage to fewer than 50,000
35    members.
