15	Between lines 1320 and 1321, insert:
16	Section 25. Subsection (6) is added to section 641.54,
17	Florida Statutes, to read:
18	641.54 Information disclosure.--
19	(6) Every health maintenance organization shall make
20	available to its subscribers the estimated co-pay, co-insurance,
21	or deductible, whichever is applicable, for any covered service,
22	the status of the subscriber’s maximum annual out-of-pocket
23	payments for a covered individual or family, and the status of
24	the subscriber’s maximum lifetime benefit. Each health
25	maintenance organization shall, upon request of a subscriber,
26	provide an estimate of the amount the health maintenance
27	organization will pay for a particular medical procedure or
28	service. The estimate may be in the form of a range of payments
29	or an average payment. A health maintenance organization that
30	provides a subscriber with a good faith estimate is not bound by
31	the estimate.
32	Section 26. If any provision of this act or the
33	application thereof to any person or circumstance is held
34	invalid, the invalidity shall not affect other provisions or
35	applications of the act which can be given effect without the
36	invalid provision or application, and to this end the provisions
37	of this act are declared severable.
