14          Amendment (with directory amendment)
15          Remove line(s) 336-370, and insert:
16          (38) "Catastrophic injury" means a permanent impairment
17    constituted by:
18          (a) Spinal cord injury involving severe paralysis of an
19    arm, a leg, or the trunk;
20          (b) Amputation of an arm, a hand, a foot, or a leg
21    involving the effective loss of use of that appendage;
22          (c) Severe brain or closed-head injury as evidenced by:
23          1. Severe sensory or motor disturbances;
24          2. Severe communication disturbances;
25          3. Severe complex integrated disturbances of cerebral
26    function;
27          4. Severe episodic neurological disorders; or
28          5. Other severe brain and closed-head injury conditions at
29    least as severe in nature as any condition provided in
30    subparagraphs 1.-4.;
31          (d) Second-degree or third-degree burns of 25 percent or
32    more of the total body surface or third-degree burns of 5
33    percent or more to the face and hands;
34          (e) Total or industrial blindness; or
35          (f) Any other injury that would otherwise qualify under
36    this chapter of a nature and severity that would qualify an
37    employee to receive disability income benefits under Title II or
38    supplemental security income benefits under Title XVI of the
39    federal Social Security Act as the Social Security Act existed
40    on July 1, 1992, without regard to any time limitations provided
41    under that act.
42          (38)(39)"Insurer" means a group self-insurers' fund
43    authorized by s. 624.4621, an individual self-insurer authorized
44    by s. 440.38, a commercial self-insurance fund authorized by s.
45    624.462, an assessable mutual insurer authorized by s. 628.6011,
46    and an insurer licensed to write workers' compensation and
47    employer's liability insurance in this state. The term
48    "carrier," as used in this chapter, means an insurer as defined
49    in this subsection.
50          (39)(40)"Statement," for the purposes of ss. 440.105 and
51    440.106, shall include the exact fraud statement language in s.
52    440.105(7). This requirementincludes, but is not limited to,
53    any notice, representation, statement, proof of injury, bill for
54    services, diagnosis, prescription, hospital or doctor record, X
55    ray, test result, or other evidence of loss, injury, or expense.
56          (40)(41)"Specificity" means information on the petition
