Remove line(s) 4607-4615, and insert:
|

attorney not later than the date of the pretrial, the benefits |
17
|
secured shall be those offered to settle each issue; however, if |
18
|
the offer is subsequently rejected by the claimant, benefits |
19
|
secured shall be only that amount awarded above that specified |
20
|
in the offer to settle. If multiple issues are pending before |
21
|
the judge of compensation claims, said offer of settlement shall |
22
|
address each issue pending, and shall state explicitly whether |
23
|
or not the offer on each issue is severable. The written offer |
24
|
shall also unequivocally state whether or not it includes |
25
|
medical witness fees and expenses, and all other costs |
26
|
associated with the claim. Acceptance of an offer by a claimant |
27
|
shall be communicated in writing to the carrier and its counsel, |
28
|
if any. The benefits contained in the offer, excluding fees and |
29
|
costs, shall be due and payable 14 days after the date the |
30
|
carrier receives the acceptance. |
