14         On page 58, line 5, after the period,

16  insert:  With respect to a determination as to whether a
17  physician or other provider has engaged in such general
18  business practice, consideration shall be given to evidence of
19  whether the physician or other provider made a good-faith
20  attempt to collect such deductible or copayment.
