14 On page 142, between lines 22 and 23,

16 insert:
17 Section 71. Paragraph (c) of subsection (2) of section
18 1006.20, Florida Statutes, as amended by section 2 of chapter
19 2003-129, Laws of Florida, is amended to read:
20 1006.20 Athletics in public K-12 schools.--
21 (2) ADOPTION OF BYLAWS.--
22 (c) The organization shall adopt bylaws that require
23 all students participating in interscholastic athletic
24 competition or who are candidates for an interscholastic
25 athletic team to satisfactorily pass a medical evaluation each
26 year prior to participating in interscholastic athletic
27 competition or engaging in any practice, tryout, workout, or
28 other physical activity associated with the student's
29 candidacy for an interscholastic athletic team. Such medical
30 evaluation can only be administered by a practitioner licensed
31 under the provisions of chapter 458, chapter 459, chapter 460,

1 or s. 464.012, and in good standing with the practitioner's
2 regulatory board. The bylaws shall establish requirements for
3 eliciting a student's medical history and performing the
4 medical evaluation required under this paragraph, which shall
5 include a physical assessment of the student's physical
6 capabilities to participate in interscholastic athletic
7 competition as contained in a uniform preparticipation
8 physical evaluation and history form. The evaluation form
9 shall incorporate the recommendations of the American Heart
10 Association for participation cardiovascular screening and
11 shall provide a place for the signature of the practitioner
12 performing the evaluation with an attestation that each
13 examination procedure listed on the form was performed by the
14 practitioner or by someone under the direct supervision of the
15 practitioner. The form shall also contain a place for the
16 practitioner to indicate if a referral to another practitioner
17 was made in lieu of completion of a certain examination
18 procedure. The form shall provide a place for the practitioner
19 to whom the student was referred to complete the remaining
20 sections and attest to that portion of the examination. The
21 preparticipation physical evaluation form shall advise
22 students to complete a cardiovascular assessment and shall
23 include information concerning alternative cardiovascular
24 evaluation and diagnostic tests. Practitioners administering
25 medical evaluations pursuant to this subsection must, at a
26 minimum, solicit all information required by, and perform a
27 physical assessment according to, the uniform preparticipation
28 form referred to in this paragraph and must certify, based on
29 the information provided and the physical assessment, that the
30 student is physically capable of participating in
31 interscholastic athletic competition. If the practitioner

1 determines that there are any abnormal findings in the
2 cardiovascular system, the student may not participate until a
3 further cardiovascular assessment, which may include an EKG,
4 is performed which indicates that the student is physically
5 capable of participating in interscholastic athletic
6 competition. Results of such medical evaluation must be
7 provided to the school. No student shall be eligible to
8 participate in any interscholastic athletic competition or
9 engage in any practice, tryout, workout, or other physical
10 activity associated with the student's candidacy for an
11 interscholastic athletic team until the results of the medical
12 evaluation clearing the student for participation has been
13 received and approved by the school.
