4     Between line 31 and 32, insert:
5     Section 2.  Subsection (1) of section 316.650, Florida
6Statutes, is amended to read:
7     316.650  Traffic citations; parental notification forms.--
8     (1)(a)  The department shall prepare, and supply to every
9traffic enforcement agency in this state, an appropriate form
10traffic citation containing a notice to appear (which shall be
11issued in prenumbered books with citations in quintuplicate) and
12meeting the requirements of this chapter or any laws of this
13state regulating traffic, which form shall be consistent with
14the state traffic court rules and the procedures established by
15the department. Upon all future printings of the traffic
16citation, the form shall include a special box which is to be
17checked by the law enforcement officer when the officer believes
18that the traffic violation or crash was due to aggressive
19careless driving as defined in s. 316.1923.
20     (b)  The department shall prepare, and supply to every
21traffic enforcement agency in the state, an appropriate
22affidavit-of-compliance form which shall be issued along with
23the form traffic citation for any violation of s. 316.610 and
24which shall indicate the specific defect which needs to be
25corrected. However, such affidavit of compliance shall not be
26issued in the case of a violation of s. 316.610 by a commercial
27motor vehicle as defined in s. 316.003(66). Such affidavit-of-
28compliance form shall be distributed in the same manner and to
29the same parties as is the form traffic citation.
30     (c)  The department shall develop a notification system
31that shall be utilized by traffic enforcement officers to assist
32in the notification of the parent of any driver under 18 years
33of age when the officer issues a citation or written warning to
34such driver for a violation of law. The notification shall
35include the time and date the vehicle was stopped, the reason
36for the stop, the number of people in the vehicle, the name of
37the person stopped, and the name of the officer. The
38notification shall be mailed by the department within 10 days to
39the address provided by the parent, guardian, or other
40responsible adult who signed the license application of the
41driver in compliance with s. 322.09(1). If no address is on file
42pursuant to s. 322.09(1)(b), the department shall mail the
43notification to the address on record for the driver. The
44department has authority to adopt rules pursuant to ss.
45120.536(1) and 120.54 to implement the provisions of this
46paragraph.
47     (d)(c)  Notwithstanding paragraphs (a), and (b), and (c), a
48traffic enforcement agency may produce uniform traffic citations
49by electronic means. Such citations must be consistent with the
50state traffic court rules and the procedures established by the
51department; must be appropriately numbered and inventoried; and
52may have fewer copies than the quintuplicate form. Affidavit-of-
53compliance forms and parental notification forms may also be
54produced by electronic means.
55     (e)(d)  The department must distribute to every traffic
56enforcement agency and to any others who request it, a traffic
57infraction reference guide describing the class of the traffic
58infraction, the penalty for the infraction, the points to be
59assessed on a driver's license, and any other information
60necessary to describe a violation and the penalties therefor.
61     Section 3.  Subsection (1) of section 322.09, Florida
62Statutes, is amended to read:
63     322.09  Application of minors; responsibility for
64negligence or misconduct of minor.--
65     (1)(a)  The application of any person under the age of 18
66years for a driver's license must be signed and verified before
67a person authorized to administer oaths by the father, mother,
68or guardian, or, if there is no parent or guardian, by another
69responsible adult who is willing to assume the obligation
70imposed under this chapter upon a person signing the application
71of a minor. This section does not apply to a person under the
72age of 18 years who is emancipated by marriage.
73     (b)  The parent, guardian, or other responsible adult
74signing the application of a minor pursuant to this section must
75furnish an address with the application for the purpose of being
76contacted in the event that the minor applicant is issued a
77written warning or a citation for a violation of law.
78     (c)(b)  There shall be submitted with each application a
79certified copy of a United States birth certificate, a valid
80United States passport, an alien registration receipt card
81(green card), an employment authorization card issued by the
82United States Department of Justice, or proof of nonimmigrant
83classification provided by the United States Department of
84Justice, for an original license.
