11  Senator Saunders moved the following amendment:

13         Senate Amendment 
14         On page 165, line 16, through
15            page 166, line 16, delete those lines
16  
17  and insert:  
18         (2)(6)  The applicant shall provide proof of liability
19  insurance as defined in s. 624.605.
20         (7)  If the applicant is a community residential home,
21  the applicant must provide proof that it has met the
22  requirements specified in chapter 419.
23         (8)>  The applicant must provide the agency with proof
24  of legal right to occupy the property.
25         (3)(9)  The applicant must furnish proof that the
26  facility has received a satisfactory firesafety inspection.
27  The local authority having jurisdiction or the State Fire
28  Marshal must conduct the inspection within 30 days after
29  written request by the applicant.
30         (4)(10)  The applicant must furnish documentation of a
31  satisfactory sanitation inspection of the facility by the

SENATE AMENDMENT Bill No. CS for CS for CS for SB 1680 Amendment No. ___ Barcode 033012 1 county health department. 2 (11) The applicant must furnish proof of compliance 3 with level 2 background screening as required under s. 4 400.4174. 5 (5)(12) A provisional license may be issued to an 6 applicant making initial application for licensure or making 7 application for a change of ownership. A provisional license 8 shall be limited in duration to a specific period of time not 9 to exceed 6 months, as determined by the agency. 10 (6)(13) A county or municipality may not issue an 11 occupational license that is being obtained for the purpose of 12 operating a facility regulated under this part without first 13 ascertaining that the applicant has been licensed to operate 14 such facility at the specified location or locations by the 15 agency.
