| 4 | Between lines 3805 and 3806 insert: |
| 5 | Section 102. The agency for Health Care Administration |
| 6 | shall contract on a statewide basis with a eye care company on a |
| 7 | capitated basis by October 1, 2004. The capitated amount shall |
| 8 | be no more than 90 percent of the current Medicaid fee for |
| 9 | service per member per month and shall cover all of the vision |
| 10 | services available to Medicaid recipients. The qualified vendor |
| 11 | must be licensed under chapter 627, Florida Statutes, have a |
| 12 | contracted statewide provider network, experience in providing |
| 13 | medical and surgical vision care for at least 5 years, and be in |
| 14 | good standing with the Agency for Health Care Administration and |
| 15 | the federal centers for Medicare and Medicaid. The agency is |
| 16 | authorized to seek Medicaid waivers or as necessary to implement |
| 17 | this program. |
