| 8 | (1) Collected by such physician or by an person or entity |
| 9 | who employs, is employed by, contracts with, or otherwise |
| 10 | provides a financial benefit to such physician; or |
| 11 | (2) Submitted by an employee or job applicant who is a |
| 12 | recipient of any medical service from such physician or from any |
| 13 | person or entity who employs, is employed by, contracts with or |
| 14 | otherwise provides a financial benefit to such physician. |
