| 4 | Remove lines 203-208 and insert: |
| 5 | any recognized hospital, or physician, or dentist, the insurer |
| 6 | shall make such payment to the designated provider of such |
| 7 | services, unless otherwise provided in the insurance contract. |
| 8 | The insurance contract may not prohibit, and claims forms must |
| 9 | provide option for, the payment of benefits directly to a |
| 10 | licensed hospital, physician, or dentist for care provided |
| 11 | pursuant to s. 395.1041. The insurer may require written |
| 12 | attestation of assignment of benefits. Payment to the provider |
| 13 | from the insurer shall be no more than the amount that the |
| 14 | insurer would otherwise have paid without the assignment. |
