4     Between line(s) 348 and 349, insert:
5     (24)  CHILD-WELFARE-TARGETED CASE MANAGEMENT.--The Agency
6for Health Care Administration, in consultation with the
7Department of Children and Family Services, may establish a
8targeted case-management project in those counties identified by
9the Department of Children and Family Services and for all
10counties with a community-based child welfare project, as
11authorized under s. 409.1671, which have been specifically
12approved by the department. Results of targeted case management
13projects shall be reported to the Social Services Estimating
14Conference established under s. 216.136. The covered group of
15individuals who are eligible to receive targeted case management
16include children who are eligible for Medicaid; who are between
17the ages of birth through 21; and who are under protective
18supervision or postplacement supervision, under foster-care
19supervision, or in shelter care or foster care, or young adults
20who have reached 18 years of age but are not yet 22 years of age
21and who were in foster care when they turned 18 years of age, as
22described in s. 409.1451. The number of individuals who are
23eligible to receive targeted case management shall be limited to
24the number for whom the Department of Children and Family
25Services has available matching funds to cover the costs. The
26general revenue funds required to match the funds for services
27provided by the community-based child welfare projects are
28limited to funds available for services described under s.
29409.1671. Additional funding for this service may be made
30available through certification of match by any local or state
31public tax revenue source. The number of individuals who are
32eligible to receive targeted case management shall be limited to
33the number for whom certified local or state public tax revenue
34sources are available to cover the cost. The Department of
35Children and Family Services may transfer the general revenue
36matching funds as billed by the Agency for Health Care
37Administration.
