12 recommended the following amendment:

14 Senate Amendment
15 On page 4, lines 2-14, delete those lines
16
17 and insert: the applicant's name, residence address, date of
18 birth, social security number, and present or former place of
19 work.
20 (7) The entry into evidence of a transaction history
21 generated by a personal identification number establishing
22 that a certification or claim for a week or weeks of benefits
23 was made against the benefit account of the individual
24 together with documentation that payment was paid by a state
25 warrant made to the order of the person or by direct deposit
26 via electronic means constitutes prima facie evidence that the
27 person claimed and received unemployment benefits from the
28 state.
29 (8) All records relating to investigations of
30 unemployment compensation fraud in the custody of the Agency
31 for Workforce Innovation or its tax collection provider are
