3     Amendment (with directory and title amendments)
4     On page 5, between lines 3 and 4, insert:
5     (20)  Any person or entity that has contracted or
6subcontracted with a licensed facility as defined in s.
7395.002(17) to receive individually identifiable patient medical
8records shall disclose to the licensed facility whether any of
9the information will be transmitted outside of the United
10States.
11     (21)  A licensed facility or a person or entity that has
12contracted or subcontracted with a licensed facility shall not
13transmit individually identifiable patient medical records to a
14site outside the United States unless the licensed facility
15discloses to the patient upon admission, or as soon as
16practicable after admission, that his or her individually
17identifiable patient medical records may be transmitted outside
18of the United States and the licensed facility obtains written
19consent from the patient to transmit his or her individually
20identifiable patient medical records to a site outside the
21United States.
