11  Senator Haridopolos moved the following amendment:

13         Senate Amendment 
14         On page 71, lines 12-27, delete those lines
15  
16  and insert:  
17         (g)  The licensing authority shall set by rule an
18  annual inspection fee not to exceed $300, payable upon
19  application for licensure and upon renewal of such licensure.
20  Centralized embalming facilities shall be subject to
21  inspection before issuance of license and annually thereafter
22  and also upon change of location and during investigation of
23  any complaint. A centralized embalming facility shall notify
24  the licensing authority of any change in location. A change in
25  ownership shall be promptly reported to the licensing
26  authority using forms and procedures specified by rule and may
27  require the relicensure of the licensee, including
28  reinspection and payment of applicable fees, as required by
29  rule. The licensing authority shall adopt rules establishing
30  inspection criteria and otherwise establishing forms and
31  procedures for the implementation of this paragraph.

 1         (i)  A change in location shall be promptly reported to
 2  the licensing authority pursuant to procedures established by
 3  rule. Operations by the licensee at a new location may not
 4  commence until an inspection by the licensing authority of the
 5  facilities, pursuant to rules of the licensing authority, has
 6  been conducted and passed at the new location.
