A bill to be entitled
2An act relating to motor vehicle registration; amending s.
3320.02, F.S.; requiring the registration application form
4to contain language permitting a voluntary contribution to
5be distributed to Southeastern Guide Dogs, Inc.; providing
6for use of the funds; exempting such funds from the
7general revenue service charge; providing an effective
8date.

10Be It Enacted by the Legislature of the State of Florida:
11
12     Section 1.  Subsections (16), (17), and (18) of section
13320.02, Florida Statutes, are amended to read:
14     320.02  Registration required; application for
15registration; forms.--
16     (16)(a)  The application form for motor vehicle
17registration shall include language permitting the voluntary
18contribution of $1 per applicant, to be quarterly distributed by
19the department to Prevent Blindness Florida, a not-for-profit
20organization, to prevent blindness and preserve the sight of the
21residents of this state. A statement providing an explanation of
22the purpose of the funds shall be included with the application
23form. Prior to the department distributing the funds collected
24pursuant to this paragraph subsection, Prevent Blindness Florida
25must submit a report to the department that identifies how such
26funds were used during the preceding year.
27     (b)(17)  The application form for motor vehicle
28registration and renewal of registration must include language
29permitting a voluntary contribution to the Florida Mothers
30Against Drunk Driving, Inc., which contribution must be
31transferred by the department to the Florida Mothers Against
32Drunk Driving, Inc., on a monthly basis.
33     (c)  The application form for motor vehicle registration
34shall include language permitting the voluntary contribution of
35$1 per applicant, to be distributed quarterly by the department
36to Southeastern Guide Dogs, Inc., a corporation not for profit
37under s. 501(c)(3) of the Internal Revenue Code, to be used by
38that organization for the purpose of breeding, raising, and
39training guide dogs for the blind. Such funds may also be used
40toward the costs of the required in-residence training for the
41individual receiving a guide dog.

43(18)  For the purpose of applying the service charge provided in
44s. 215.20, contributions received under this subsection
45subsections (16) and (17) are not income of a revenue nature.
46     Section 2.  This act shall take effect July 1, 2005.
