15         On page 5, line 31, delete that line

17  and insert:  requesting access; which involve issues relating
18  to a provider's competence, qualifications, and experience,
19  including, but not limited to, substance abuse, allegations or
20  findings of misconduct, or the performance of a procedure or
21  treatment on the wrong part of a patient or on the wrong
22  patient; which involve a facility's experience with issues
23  that are not specific to a patient's condition or treatment,
24  including, but not limited to, infection experiences,
25  medication, radiology, pathology, or anesthesia incidents; or
26  which involve patient supervision and assistance.
