11  The Committee on Ways and Means (Campbell) recommended the
12  following amendment:

14         Senate Amendment 
15         On page 7, line 30,
16  
17  insert:  
18         (i)1.  Procedures to require accurate reporting to the
19  division of injuries incurred by greyhounds that race at any
20  facility that is licensed as a slot machine facility in this
21  state. The reports shall include:
22         a.  The greyhound's registered name and right and left
23  ear tattoo numbers;
24         b.  The name, business address, and telephone number of
25  the owner, trainer, and kennel operator of the greyhound;
26         c.  The color, weight, and sex of the greyhound;
27         d.  The specific type of injury, cause of the injury,
28  estimated recovery time, and location of the injury on the
29  greyhound;
30         e.  Where the injury occurred, whether on a racing
31  track or in another area;

If the injury occurred while the greyhound was 2 racing, the racetrack where the injury occurred; the distance, 3 grade, race, and post position of the greyhound when the 4 injury occurred; and the weather conditions, time, 5 temperature, and track condition at the time of injury; and 6 g. A certification by the racetrack veterinarian that 7 the form is correct. 8 2. The division shall maintain injury records for a 9 period of 7 years. 10 3. In addition to other penalties imposed by law, a 11 person who knowingly makes a false statement on an injury 12 report commits a misdemeanor of the first degree, for a first 13 violation, punishable as provided in s. 775.082 or s. 775.083, 14 and a felony of the third degree, for a subsequent violation, 15 punishable as provided in s. 775.082, s. 775.083, or s. 16 775.084. 17 18 19 20 21 22 23 24 25 26 27 28 29 30 31 2 2:46 PM 12/06/05 s0004Bb-wm32-c6g
