15 On page 9, between lines 25 and 26,

17 insert:
18 (16) "Child's history record" means a written record
19 prepared and maintained by the department or its contracted
20 community-based care provider on behalf of each child in its
21 custody, including those in shelter or foster care or
22 otherwise. The record includes, at a minimum, the child's
23 name; family and social history; educational records and
24 history; history of placements within state custody; medical
25 history, including the respective dates and purposes of
26 medical care and treatment, the results of all medical,
27 psychiatric, and psychological information, evaluations,
28 exams, educational and social needs assessments, visits,
29 hospitalizations, operations, and procedures, with reasons
30 therefor; the dates and locations of the treatment and the
31 names of treating or administering professionals; the names

1 and telephone numbers of all physicians and other health care
2 professionals who have treated the child; the child's known
3 allergies and negative reactions to medication; all
4 medications previously and currently prescribed, including
5 dates of administration, re-prescription, and discontinuation,
6 the dosage and frequency, and subsequent re-prescription; any
7 known side effects or contraindications; any written express
8 and informed consents as required by law; the name and phone
9 number of a department agent who is currently responsible for
10 the child; the name and phone number of the parent, legal
11 custodian, relative caregiver, or foster care parent, if
12 applicable; and the local after-hour call number for the
13 department for emergencies.
