4	On page 5, between lines 20 and 21, insert:
5	Section 7.  Subsection (3) is added to section 627.6402,
6	Florida Statutes, to read:
7	627.6402  Insurance rebates for healthy lifestyles.--
8	(3)  In addition to the outline of coverage, a policy as
9	specified in s. 627.6699(3)(k) must be accompanied by an
10	identification card that contains, at a minimum:
11	(a)  The name of the organization issuing the policy or
12	the name of the organization administering the policy, whichever
13	applies.
14	(b)  The name of the contract holder.
15	(c)  The type of plan only if the plan is filed in the
16	state, an indication that the plan is self-funded, or the name
17	of the network.
18	(d)  The member identification number, contract number,
19	and policy or group number, if applicable.
20	(e)  A contact phone number or electronic address for
21	authorizations.
22	(f)  A phone number or electronic address by which the
23	covered person or hospital, physician, or other person rendering
24	services covered by the policy may determine if the plan is
25	insured and may obtain a benefits verification in order to
26	estimate patient financial responsibility, in compliance with
27	privacy rules under the Health Insurance Portability and
28	Accountability Act of 1996.
29	(g)  The national plan identifier, in accordance with the
30	compliance date set forth by the United States Department of
31	Health and Human Services. The identification card must present
32	the information in a readily identifiable manner or the
33	information may be embedded on the card and available through
34	magnetic stripe or smart card. The information may also be
35	provided through other electronic technology.
36	Section 8.  Subsection (2) of section 627.657, Florida
37	Statutes, is renumbered as subsection (3), and a new subsection
38	(2) is added to that section, to read:
39	627.657  Provisions of group health insurance policies.--
40	(2)  The medical policy as specified in s. 627.6699(3)(k)
41	must be accompanied by an identification card that contains, at
42	a minimum:
43	(a)  The name of the organization issuing the policy or
44	name of the organization administering the policy, whichever
45	applies.
46	(b)  The name of the certificateholder.
47	(c)  The type of plan only if the plan is filed in the
48	state, an indication that the plan is self-funded, or the name
49	of the network.
50	(d)  The member identification number, contract number,
51	and policy or group number, if applicable.
52	(e)  A contact phone number or electronic address for
53	authorizations.
54	(f)  A phone number or electronic address by which the

73	(41)  The contract, certificate, or member handbook must
74	be accompanied by an identification card that contains, at a
75	minimum:
76	(a)  The name of the organization offering the contract or
77	name of the organization administering the contract, whichever
78	applies.
79	(b)  The name of the subscriber.
80	(c)  A statement that the health plan is a health
81	maintenance organization. Only a health plan with a certificate
82	of authority issued under this chapter may be identified as a
83	health maintenance organization.
84	(d)  The member identification number, contract number,
85	and group number, if applicable.
86	(e)  A contact phone number or electronic address for
87	authorizations.
88	(f)  A phone number or electronic address by which the
89	covered person or hospital, physician, or other person rendering
90	services covered by the contract may determine if the plan is
91	insured and may obtain a benefits verification in order to
92	estimate patient financial responsibility, in compliance with
93	privacy rules under the Health Insurance Portability and
94	Accountability Act of 1996.
