3     Amendment (with title amendment)
4     On page 5, between lines 20 and 21, insert:
5     Section 7.  Subsection (3) is added to section 627.6402,
6Florida Statutes, to read:
7     627.6402  Insurance rebates for healthy lifestyles.--
8     (3)  In addition to the outline of coverage, a policy as
9specified in s. 627.6699(3)(k) must be accompanied by an
10identification card that contains, at a minimum:
11     (a)  The name of the organization issuing the policy or
12the name of the organization administering the policy, whichever
13applies.
14     (b)  The name of the contract holder.
15     (c)  The type of plan only if the plan is filed in the
16state, an indication that the plan is self-funded, or the name
17of the network.
18     (d)  The member identification number, contract number,
19and policy or group number, if applicable.
20     (e)  A contact phone number or electronic address for
21authorizations.
22     (f)  A phone number or electronic address by which the
23covered person or hospital, physician, or other person rendering
24services covered by the policy may determine if the plan is
25insured and may obtain a benefits verification in order to
26estimate patient financial responsibility, in compliance with
27privacy rules under the Health Insurance Portability and
28Accountability Act of 1996.
29     (g)  The national plan identifier, in accordance with the
30compliance date set forth by the United States Department of
31Health and Human Services. The identification card must present
32the information in a readily identifiable manner or the
33information may be embedded on the card and available through
34magnetic stripe or smart card. The information may also be
35provided through other electronic technology.
36     Section 8.  Subsection (2) of section 627.657, Florida
37Statutes, is renumbered as subsection (3), and a new subsection
38(2) is added to that section, to read:
39     627.657  Provisions of group health insurance policies.--
40     (2)  The medical policy as specified in s. 627.6699(3)(k)
41must be accompanied by an identification card that contains, at
42a minimum:
43     (a)  The name of the organization issuing the policy or
44name of the organization administering the policy, whichever
45applies.
46     (b)  The name of the certificateholder.
47     (c)  The type of plan only if the plan is filed in the
48state, an indication that the plan is self-funded, or the name
49of the network.
50     (d)  The member identification number, contract number,
51and policy or group number, if applicable.
52     (e)  A contact phone number or electronic address for
53authorizations.
54     (f)  A phone number or electronic address by which the
55covered person or hospital, physician, or other person rendering
56services covered by the policy may determine if the plan is

71Florida Statutes, to read:
72     641.31  Health maintenance contracts.--
73     (41)  The contract, certificate, or member handbook must
74be accompanied by an identification card that contains, at a
75minimum:
76     (a)  The name of the organization offering the contract or
77name of the organization administering the contract, whichever
78applies.
79     (b)  The name of the subscriber.
80     (c)  A statement that the health plan is a health
81maintenance organization. Only a health plan with a certificate
82of authority issued under this chapter may be identified as a
83health maintenance organization.
84     (d)  The member identification number, contract number,
85and group number, if applicable.
86     (e)  A contact phone number or electronic address for
87authorizations.
88     (f)  A phone number or electronic address by which the
89covered person or hospital, physician, or other person rendering
90services covered by the contract may determine if the plan is
91insured and may obtain a benefits verification in order to
92estimate patient financial responsibility, in compliance with
93privacy rules under the Health Insurance Portability and
94Accountability Act of 1996.
95     (g)  The national plan identifier, in accordance with the
