11  The Committee on Health Care (Fasano) recommended the
12  following amendment:

14         Senate Amendment (with title amendment) 
15         On page 3, between lines 15 and 16,
16  
17  insert:  
18         Section 1.  Subsection (2) of section 316.068, Florida
19  Statutes, is amended to read:
20         316.068  Crash report forms.--
21         (2)  Every crash report required to be made in writing
22  must be made on the appropriate form approved by the
23  department and must contain all the information required
24  therein, including the date, time, and location of the crash;
25  a description of the vehicles involved; the names and
26  addresses of the parties involved; the names and addresses of
27  all drivers and passengers in the vehicles involved; the names
28  and addresses of witnesses; the name, badge number, and law
29  enforcement agency of the officer investigating the crash; and
30  the names of the insurance companies for the respective
31  parties involved in the crash, unless not available. The
