11  The Committee on Health Care (Fasano) recommended the
12  following amendment:
13  
14         Senate Amendment (with title amendment) 
15         On page 10, between lines 20 and 21,
16  
17  insert:  
18         Section 11.  Subsection (4) of section 627.4133,
19  Florida Statutes, is amended to read:
20         627.4133  Notice of cancellation, nonrenewal, or
21  renewal premium.--
22         (4)  Notwithstanding the provisions of s. 440.42(3), if
23  cancellation of a policy providing coverage for workers'
24  compensation and employer's liability insurance is requested
25  by the insured, such cancellation shall be effective on the
26  date the carrier sends the notice of cancellation to the
27  insured. Any retroactive assumption of coverage and
28  liabilities under a policy providing workers' compensation and
29  employer's liability insurance may not exceed 21 days.
