14 Senate Amendment (with title amendment)
15 On page 27, line 22, through
16 page 29, line 22, delete those lines
17
18 and insert:
19 (10)(9) Any person or entity providing health care
20 services which is not a clinic or specialty clinic, as defined
21 under s. 400.9905, may voluntarily apply for a certificate of
22 exemption from licensure under its exempt status with the
23 agency on a form that sets forth its name or names and
24 addresses, a statement of the reasons why it cannot be defined
25 as a clinic, and other information deemed necessary by the
26 agency. An exemption is not transferable. The agency may
27 charge an applicant for a certificate of exemption in an
28 amount equal to $100 or the actual cost of processing the
29 certificate, whichever is less.
