11  The Committee on Health Care (Jones) recommended the following
12  amendment:

14         Senate Amendment (with title amendment) 
15         On page 16, between lines 17 and 18,

17  insert:  
18         Section 4.  Paragraph (hh) is added to subsection (1)
19  of section 456.072, Florida Statutes, to read:
20         456.072  Grounds for discipline; penalties;
21  enforcement.--
22         (1)  The following acts shall constitute grounds for
23  which the disciplinary actions specified in subsection (2) may
24  be taken:
25         (hh)  Intentionally providing false information on an
26  application for a certificate of exemption from clinic
27  licensure under part XIII of chapter 400.
