11  The Committee on Health Care (Jones) recommended the following
12  amendment:

14         Senate Amendment 
15         On page 12, lines 28-29, delete those lines
16  
17  and insert:  licensure under its exempt status with the agency
18  on a form that sets forth its name or names and addresses, a
19  statement of the reasons why it cannot be defined as a clinic,
20  and other information deemed necessary by the agency. An
21  exemption is not transferable. The agency may charge an
22  applicant for a certificate of exemption in an amount equal to
23  $100 or the actual cost of processing the certificate,
24  whichever is less. with the agency on
