11 The Committee on Health Care (Saunders) recommended the
12 following amendment:

15 On page 17, lines 5-9, delete those lines
16
17 and insert:
18 2. If the insured fails to furnish the provider with
19 the correct name and address of the insured's personal injury
20 protection insurer, the provider has 75 days following the
21 date the provider obtains the correct information to furnish
22 the insurer with a statement of the charges. The insurer is
23 not required to pay for such charges unless the provider
24 includes with the statement:
25 a. Documentary evidence that was provided by the
26 insured during the 75-day period demonstrating that the
27 provider reasonably relied on erroneous information from the
28 insured;
29 b. A denial letter from the incorrect insurer; or
30 c. Proof of mailing, which may include an affidavit
31 under penalty of perjury, reflecting timely mailing to the
