A bill to be entitled
2An act relating to motor vehicle registration; amending s.
3320.02, F.S.; requiring that the application forms for
4registration and renewal contain a provision permitting a
5voluntary contribution to be distributed to Florida Heart
6Research Institute, Inc.; providing for use of the funds;
7exempting such funds from the general revenue service
8charge; providing an effective date.

10Be It Enacted by the Legislature of the State of Florida:
11
12     Section 1.  Subsection (16) of section 320.02, Florida
13Statutes, is amended to read:
14     320.02  Registration required; application for
15registration; forms.--
16     (16)(a)  The application form for motor vehicle
17registration shall include language permitting the voluntary
18contribution of $1 per applicant, to be quarterly distributed by
19the department to Prevent Blindness Florida, a not-for-profit
20organization, to prevent blindness and preserve the sight of the
21residents of this state. A statement providing an explanation of
22the purpose of the funds shall be included with the application
23form. Prior to the department distributing the funds collected
24pursuant to this paragraph, Prevent Blindness Florida must
25submit a report to the department that identifies how such funds
26were used during the preceding year.
27     (b)  The application form for motor vehicle registration
28and renewal of registration must include language permitting a
29voluntary contribution to the Florida Mothers Against Drunk
30Driving, Inc., which contribution must be transferred by the
31department to the Florida Mothers Against Drunk Driving, Inc.,
32on a monthly basis.
33     (c)  The application form for motor vehicle registration
34shall include language permitting the voluntary contribution of
35$1 per applicant, to be distributed quarterly by the department
36to Southeastern Guide Dogs, Inc., a corporation not for profit
37under s. 501(c)(3) of the Internal Revenue Code, to be used by
38that organization for the purpose of breeding, raising, and
39training guide dogs for the blind. Such funds may also be used
40toward the costs of the required in-residence training for the
41individual receiving a guide dog.
42     (d)  The application form for motor vehicle registration
43and renewal of registration shall include language permitting a
44voluntary contribution of $1 to "Stop Heart Disease." Such funds
45shall be distributed quarterly by the department to the Florida
46Heart Research Institute, Inc., a corporation not for profit
47under s. 501(c)(3) of the Internal Revenue Code. Funds shall be
48used by the organization for the purpose of heart disease
49research, education, and prevention programs.
50     (e)  For the purpose of applying the service charge
51provided in s. 215.20, contributions received under this
52subsection are not income of a revenue nature.
53     Section 2.  This act shall take effect July 1, 2006.
