6An act relating to motor vehicle registration forms;
7amending s. 320.02, F.S.; requiring the application form
8to be revised to allow each applicant to make a voluntary
9contribution to the Miami Heart Research Institute, Inc.,
10doing business as the Florida Heart Research Institute;
11providing an effective date.

13Be It Enacted by the Legislature of the State of Florida:
14
15     Section 1.  Subsection (16) of section 320.02, Florida
16Statutes, is amended to read:
17     320.02  Registration required; application for
18registration; forms.--
19     (16)(a)  The application form for motor vehicle
20registration shall include language permitting the voluntary
21contribution of $1 per applicant, to be quarterly distributed by
22the department to Prevent Blindness Florida, a not-for-profit
23organization, to prevent blindness and preserve the sight of the
24residents of this state. A statement providing an explanation of
25the purpose of the funds shall be included with the application
26form. Prior to the department distributing the funds collected
27pursuant to this paragraph, Prevent Blindness Florida must
28submit a report to the department that identifies how such funds
29were used during the preceding year.
30     (b)  The application form for motor vehicle registration
31and renewal of registration must include language permitting a
32voluntary contribution to the Florida Mothers Against Drunk
33Driving, Inc., which contribution must be transferred by the
34department to the Florida Mothers Against Drunk Driving, Inc.,
35on a monthly basis.
36     (c)  The application form for motor vehicle registration
37shall include language permitting the voluntary contribution of
38$1 per applicant, to be distributed quarterly by the department
39to Southeastern Guide Dogs, Inc., a corporation not for profit
40under s. 501(c)(3) of the Internal Revenue Code, to be used by
41that organization for the purpose of breeding, raising, and
42training guide dogs for the blind. Such funds may also be used
43toward the costs of the required in-residence training for the
44individual receiving a guide dog.
45     (d)  The application form for motor vehicle registration
46shall include language permitting the voluntary contribution of
47$1 per applicant, to be distributed quarterly by the department
48to the Miami Heart Research Institute, Inc., doing business as
49the Florida Heart Research Institute, a corporation not for
50profit under s. 501(c)(3) of the Internal Revenue Code, to be
51used by that organization for the purpose of heart-disease
52research, education, and prevention programs.
53     (e)  For the purpose of applying the service charge
54provided in s. 215.20, contributions received under this
55subsection are not income of a revenue nature.
56     Section 2.  This act shall take effect July 1, 2006.
