4     Remove line(s) 107-167 and insert:
5     Section 3.  Subsections (5), (6), and (7) of section
6381.0056, Florida Statutes, are amended to read:
7     381.0056  School health services program.--
8     (5)(a)  Each county health department shall develop,
9jointly with the district school board and the local school
10health advisory committee, a school health services plan; and
11the plan must shall include, at a minimum, provisions for:
12     1.(a)  Health appraisal;
13     2.(b)  Records review;
14     3.(c)  Nurse assessment;
15     4.(d)  Nutrition assessment;
16     5.(e)  A preventive dental program;
17     6.(f)  Vision screening;
18     7.(g)  Hearing screening;
19     8.(h)  Scoliosis screening;
20     9.(i)  Growth and development screening;
21     10.(j)  Health counseling;
22     11.(k)  Referral and followup of suspected or confirmed
23health problems by the local county health department;
24     12.(l)  Meeting emergency health needs in each school;
25     13.(m)  County health department personnel to assist school
26personnel in health education curriculum development;
27     14.(n)  Referral of students to appropriate health
28treatment, in cooperation with the private health community
29whenever possible;
30     15.(o)  Consultation with a student's parent or guardian
31regarding the need for health attention by the family physician,
32dentist, or other specialist when definitive diagnosis or
33treatment is indicated;
34     16.(p)  Maintenance of records on incidents of health
35problems, corrective measures taken, and such other information
36as may be needed to plan and evaluate health programs; except,
37however, that provisions in the plan for maintenance of health
38records of individual students must be in accordance with s.
391002.22;
40     17.(q)  Health information which will be provided by the
41school health nurses, when necessary, regarding the placement of
42students in exceptional student programs and the reevaluation at
43periodic intervals of students placed in such programs; and
44     18.(r)  Notification to the local nonpublic schools of the
45school health services program and the opportunity for
46representatives of the local nonpublic schools to participate in
47the development of the cooperative health services plan.
48     (b)  Each school health advisory committee must, at a
49minimum, include members who represent the eight component areas
50of the Coordinated School Health model as defined by the Centers
51for Disease Control and Prevention. School health advisory
52committees are encouraged to address the eight components of the
53Coordinated School Health model in the school district's school
54wellness policy pursuant to s. 1003.453.
55     (6)  A nonpublic school may request to participate in the
56school health services program. A nonpublic school voluntarily
57participating in the school health services program shall:
58     (a)  Cooperate with the county health department and
59district school board in the development of the cooperative
60health services plan;
61     (b)  Make available adequate physical facilities for health
62services;
63     (c)  Provide inservice health training to school personnel;
64     (d)  Cooperate with public health personnel in the
65implementation of the school health services plan;
66     (e)  Be subject to health service program reviews by the
67Department of Health and the Department of Education; and
68     (f)  At the beginning of each school year, provide parents
69and guardians with information concerning ways that they can
70help their children to be physically active and to eat healthful
71foods; and
72     (g)(f)  At the beginning of each school year, inform
73parents or guardians in writing that their children who are
74students in the school will receive specified health services as
75provided for in the district health services plan. A student
76will be exempt from any of these services if his or her parent
77or guardian requests such exemption in writing. This paragraph
78shall not be construed to authorize invasive screening; if there
79is a need for such procedure, the consent of the student's
80parent or guardian shall be obtained in writing prior to
81performing the screening. However, the laws and rules relating
82to contagious or communicable diseases and sanitary matters
83shall not be violated.
84     (7)  The district school board shall:
85     (a)  Include health services and health education as part
86of the comprehensive plan for the school district;
87     (b)  Provide inservice health training for school
88personnel;
89     (c)  Make available adequate physical facilities for health
90services; and
91     (d)  At the beginning of each school year, provide parents
92and guardians with information concerning ways that they can
93help their children to be physically active and to eat healthful
94foods; and
95     (e)(d)  At the beginning of each school year, inform
96parents or guardians in writing that their children who are
97students in the district schools will receive specified health
98services as provided for in the district health services plan. A
99student will be exempt from any of these services if his or her
100parent or guardian requests such exemption in writing. This
101paragraph shall not be construed to authorize invasive
102screening; if there is a need for such procedure, the consent of
103the student's parent or guardian shall be obtained in writing
