12 recommended the following amendment:

14 Senate Amendment (with title amendment)
15 On page 7, between lines 11 and 12,

17 insert:
18 Section 5. Subsection (2) of section 493.6406, Florida
19 Statutes, is amended to read:
20 493.6406 Repossession services school or training
21 facility.--
22 (2) The application shall be signed and notarized and
23 shall contain, at a minimum, the following information:
24 (a) The name and address of the school or training
25 facility and, if the applicant is an individual, his or her
26 name, address, and social security or alien registration
27 number.
28 (b) The street address of the place at which the
29 training is to be conducted.
30 (b)(c) A copy of the training curriculum and final
31 examination to be administered.
