3     Amendment to Amendment (194097)
4Remove lines 644-648 and insert:
5     2.  If the insured fails to furnish the provider with the
6correct name and address of the insured's personal injury
7protection insurer, the provider has 75 days from the date the
8provider obtains the correct information to furnish the insurer
9with a statement of the charges.  The insurer is not required to
10pay for such charges unless the provider includes with the
11statement documentary evidence that was provided by the insured
12during the 75-day period demonstrating that the provider
13reasonably relied on erroneous information from the insured or:
14     a.  A denial letter from the incorrect insurer; or
15     b.  Proof of mailing, which may include an affidavit under
16penalty of perjury, reflecting timely mailing to the incorrect
17address or insurer.
