  1                      A bill to be entitled

  2         An act relating to motor vehicle registration

  3         forms; requiring the application form to be

  4         revised to allow each applicant to make a

  5         contribution to the Florida Heart Research

  6         Institute, Inc.; providing an effective date.

  8  Be It Enacted by the Legislature of the State of Florida:

  9  

10         Section 1.  Paragraph (d) is added to subsection (16)

11  of section 320.02, Florida Statutes, to read:

12         320.02  Registration required; application for

13  registration; forms.--

14         (16)

15         (d)  The application form for motor vehicle

16  registration shall include language permitting the voluntary

17  contribution of $1 per applicant, to be distributed quarterly

18  by the department to the Florida Heart Research Institute,

19  Inc., a corporation not for profit under s. 501(c)(3) of the

20  Internal Revenue Code, to be used by that organization for the

21  purpose heart disease research, education, and prevention

22  programs.

23         Section 2.  This act shall take effect July 1, 2007.
