  1                      A bill to be entitled

  2         An act relating to motor vehicle registration

  3         forms; requiring the application form to be

  4         revised to allow each applicant to make a

  5         contribution to the Miami Heart Research

  6         Institute, Inc., doing business as the Florida

  7         Heart Research Institute; providing an

  8         effective date.

10  Be It Enacted by the Legislature of the State of Florida:

11  

12         Section 1.  Paragraph (d) is added to subsection (16)

13  of section 320.02, Florida Statutes, to read:

14         320.02  Registration required; application for

15  registration; forms.--

17         (d)  The application form for motor vehicle

18  registration shall include language permitting the voluntary

19  contribution of $1 per applicant, to be distributed quarterly

20  by the department to the Miami Heart Research Institute, Inc.,

21  doing business as the Florida Heart Research Institute, a

22  corporation not for profit under s. 501(c)(3) of the Internal

23  Revenue Code, to be used by that organization for the purpose

24  of heart-disease research, education, and prevention programs.

25         Section 2.  This act shall take effect July 1, 2006.

 1          STATEMENT OF SUBSTANTIAL CHANGES CONTAINED IN
                       COMMITTEE SUBSTITUTE FOR
 2                         Senate Bill 738

 3                                 

 4  The committee substitute requires the Department of Highway
    Safety and Motor Vehicles to include language allowing a
 5  voluntary $1 contribution to the Miami Heart Research
    Institute, Inc., doing business as the Florida Heart Research
 6  Institute on each motor vehicle registration and renewal form.
    The committee substitute also changes the effective date from
 7  July 1, 2007, to July 1, 2006.
