15         On page 5, lines 21-23, delete those lines

17  and insert:  
18         (7)  Only a parent named on a fetal death certificate
19  may request that the Office of Vital Statistics create a
20  certificate of birth resulting in stillbirth. The Office of
21  Vital Statistics must inform any parent who requests a
22  certificate of birth resulting in stillbirth that a copy of
23  the document is available as a public record when held by an
24  agency as defined under s. 119.011(2). A refusal by the Office
